Below are the documents that need to be filled out to register your child for school. Many of these documents are good for students k - 12.

You can type in each document but it cannot be saved. You must print the document prior to closing the document or you will loose all the information.

Parent Information

Please bring the completed form along with:

• 2 proofs of residency (telephone bill, electric bill, water bill, etc.) (mandatory)
• Child's birth certificate (mandatory)
• Copy of child's medical records (mandatory)
• Copy of any early intervention or special education evaluations (optional)

Below is the only additional form for incoming Preschool or Kindergarten families. It is to be completed by student’s current preschool/childcare provider. Print it out and give to your child's teacher to fill out. Have them mail the completed document to the school your child will attend.
